- Note: requests for new accounts that will be funded by grants should be requested through ORA.
Required Fields:
| Type of account | Check appropriate box |
| Short Description | This is the account name that will appear on ledger sheets and all printed reports. Can contain abbreviations or acronyms. |
| Long Description | Additional account name or description information. Cannot contain abbreviations or acronyms. Will appear on a screen in the accounting system. |
| Purpose or justification | Indicate what account will be used for, how long account is needed for, how the account will be funded, and any other pertinent information. |
| Box # | Indicate box number ledger sheets should be mailed to |
| Responsible person | Last name, first initial. |
| Dept name / Number | Department name and 5-digit department number |
| Other info | Fill in if known; otherwise Controller's Office will complete. |

Form should be forwarded to the Controller's Office Data Control Area at Box J or e-mailed to datacntl@brown.edu
