Applying Without the Committee Letter
If you do not meet the eligibility requirements for the committee letter the Office of the Dean of the College will be happy to serve as a credential service to hold and send out your individual letters of recommendation for application to medical or other health profession schools.
You will need to submit the following:
- One copy of the Health Careers Advising Dossier (HCAD) (Activities and Personal Statement sections are optional)
- List of Letter Writers & Waiver Form (use additional forms if you are submitting more than three recommendations—5 recommendation limit )
- AMCAS (or other) Application with Processing Date
- Fee Payment of $50.00. Current students may pay by check or can have the fee charged to their student account. Alumni must pay by check.
- Mailing Form
Prior to submitting these items, applicants should make an appointment to speak with Dean Simmons either in person or on the phone.
