Dependent Eligibility and Enrollment
Insured students may enroll their eligible dependent(s) in the Student Health Insurance Plan at an additional cost. Dependent means: the spouse (husband or wife or domestic partner) of the named insured and their dependent, unmarried children. Children shall cease to be dependent on the first to occur of: 1) the end of the month in which they marry; or 2) the end of the month in which they attain the age of nineteen (19) or twenty-three (23) years, if a full-time dependent student at an accredited institution of higher learning. Newborn infant means any child born of any insured while that person is insured under this policy. Please refer to the 2008 - 2009 plan brochure for important information regarding coverage for newborns.
Domestic partner means the partner of an insured student who has filed a "Declaration of Domestic Partnership" (same-sex partners) or a "Common Law Marriage Affidavit" (opposite-sex partners) with the Insurance Office. Please call 401-863-1703 for further information.
Eligible dependents must be added by the following enrollment deadlines: September 15, 2008 for an effective date of August 15, 2008; February 15, 2009 for an effective date of January 15, 2009; and July 1, 2009 for an effective date of June 1, 2009. Outside of these enrollment periods, eligible dependents can only be added to the plan within 31 days of a qualifying event, such as marriage, spouse's initial arrival to the United States, birth of a child, loss of coverage due to a change in employment status, etc. Please be advised that premiums are not pro-rated for enrollment forms received after these deadlines.

Students also have the option of enrolling their eligible dependents online at www.universityhealthplans.com.
It is the student's responsibility to ensure the timely enrollment of eligible dependents each policy year. Previously insured dependents must be re-enrolled by the deadlines in order to avoid a break in coverage. Dependents are not automatically re-enrolled in the plan.
