Mumps

Clinical Features:

 

        Prodrome ñ Incubation period 14-18 days

İNon-specific symptoms, including headache, low grade fever,

İ malaise, anorexia

        Parotitisİ -- Inflammation of the Parotid gland

İİİİİİİİİİİİİİİİİİ Most common manifestation

İİİİİ İİİİİ İOccurs in 30-40% of infected patients

İİİİİ İİİİİ İTends to occur in first 2 days

İİİİİ İİİİİ İNoted as earache or tenderness at angle of jaw

Can be unilateral or bilateral; can affect single or multiple glands

İİİİİİİİİİİİİİİİİİ Symptoms being to decrease after 1 week

İİİİİİİİİİİ ** 20% of Mumps infections are asymptomatic, and another 40-50% may

İİİİ have only nonspecific symptoms (i.e. cold-like)

 

Complications:

 

        Central Nervous System (CNS) involvement --

Aseptic meningitis: asymptomatic, in 50-60% of patients

Symptomatic meningitis: resolves without sequelae in 3-10 days, in 15%

of patients;İ boys affected more commonly than girls (3:1 ratio);

parotitis absent in 50% of such cases

 

        Orchitis ñ testicular inflammation, in 20-50% of postpubertal males

 

        Oophoritis ñ ovarian inflammation, in 5% of postpubertal females

 

        Pancreatitis ñ in 2-5% of reported cases, usually occurs without parotitis;

İİİİİİ association with Diabetes Mellitus, but no causal relationship determined

 

        Deafness ñ 1 in 20,000 reported cases; one of leading causes of acquired

İİİİİİ sensorineural deafness in childhood; unilateral in 80% of cases

 

        Death ñ 1-3 per 10,000 cases in recent years
