  • Adult Day Services:

    Licensed centers provide frail and functionally challenged adults, including those with Alzheimer’s Disease and related Dementia, with care and supervision in a safe environment.  Services include therapeutic, recreation, health services, and respite for caregivers. Payment for services are based on income. Eligibility for co-pay programs is determined by the local center.

    Case Management Programs:

    Case managers from local community agencies work with elders 60 or older to assess the elders needs and develop a plan to help homebound, frail or disabled seniors remain home with supportive care. Based on eligibility, home health aide or homemaker services, adult day services, meals on wheels or senior companion services may be available through the Department of Elderly Affairs.

    Geriatric Care Managers, (617)426-3533:

    Specialists who assist seniors and their families to plan for and implement ways to allow for the greatest degree of independence safety and comfort.

    • Legal Terms:
      • Advance Directives-written instructions indicating the medical care that persons wish or decline if unable to communicate their wishes, may be revoked at any time.

     

      • Durable Power of Attorney for Health Care- allows persons to appoint an agent to make health care decisions on the person’s behalf. The agent has the legal authority to make the medical treatment choices a person would have made for themselves if able to do so.

     

      • Living Will- Individuals direct their physicians as to which life-sustaining medical procedures they wish to utilize or stop, if such procedures only serve to prolong the dying process.

     

        • Paperwork and requirements will vary in each state. You should execute your advanced directives for your state of residence.
     

    Medicare Information:

    A person becomes eligible for Medicare 3 months before the month of their 65th birthday until 3 months after their 65th birthday.  A financial penalty may be imposed if a person doesn’t enroll when becoming eligible. Below is some general information on the 4 categories of Medicare, visit the Medicare website at www.medicare.gov or call the local Department of Elderly Affairs for more information.

     

    • Part A- Premium free portion of Medicare, covering hospital stays at skilled nursing facilities, home health care services, psychiatric and hospice inpatient care, up to a 60 day stay.

     

    • Part B-Medical insurance portion of Medicare.  Covers physician services and outpatient hospital care.  Monthly premiums are paid by enrollees and by contributions by the federal government.  Deductible paid by enrollees.

     

    • Part C- Medicare Advantage available for persons who are eligible for A but enrolled in B.  Private health insurance companies can contract with the federal government to offer Medicare benefits through their own policies.  Through managed care companies will offer benefits at lower costs and with benefits not typically offered through Medicare.

     

    • Part D-Prescription Drug Coverage covering both brand name and generic prescription drugs through participating pharmacies.  Everyone with Medicare is eligible for this plan regardless of income, resources, health status or current prescription expenses.  Enrollees are responsible for a monthly premium and part of the prescription costs.
