DONOR CALL LIST

1. Primary Doctor: ___________________________________
This is the physician who took care of the donor for the longest time. This may be the neurologist who originally diagnosed your family member with Alzheimer's Disease.


2. Hospital or Nursing Home Doctor___________________________________
Care may have been transfered recently or years ago. In either case, the full medical history, including the last few days of life, are important.


3. Nursing Supervisor/House Staff Doctor: ____________________________________
INFORM the doctors in the nursing home or hospital ward who are directly involved in the care of your family member. Tell them upon admission that your next-of-kin is a brain donor and that an authorization for release of medical records was already signed. They may ask you to sign a second form from the facility.

 

4. Funeral Home___________________________________

INFORM Funeral Home that your family member is a brain donor. Speak with the funeral director and make sure he/she is familiar with organ/brain donation procedures. It is important in case the funeral home receives the body first. Transport to the Rhode Island Hospital must occur SOON AFTER DEATH and BEFORE embalming. The embalming chemicals are not meant to preserve organ tissue for pathological diagnosis and for research. The hospital/Nursing home may call the funeral home for you and even arrange for all the transportation. Know in advance what to exptect.
