Health History Form
Your Health History Form is due on July 1st. The information
on this form provides our staff with an overview of your health
status and past medical history. This assists us in providing the
best care possible, and to meet your health care needs. All medical
information is strictly confidential, and cannot be released to
anyone (including your parents, professors) without your authorization.

The health form also provides us with your immunization
history. The following immunizations are required by Rhode Island
State law and Brown University for all students:
- All students who enter a college or university
are required to have received two (2) doses of a measles-containnig
vaccine (preferably MMR vaccine), and one (1) dose of a rubella
vaccine. The first dose of these three vaccines must be given
on or after the first birthday. The second dose of a measles-contraining
vaccine (preferably MMR vaccine) must be give at least one (1)
month after the first dose.
- Tetanus / Diphtheria booster within the past 10
years
- Hepatitis B vaccine - three doses required (or
at least one dose to start the series; remaining doses can be
done with private physician or with the Nursing Staff at Health
Services)
- PPD / Mantoux skin test - within past 6 months
- Varicella (chickenpox) - history of disease or
vaccine, two doses
Brown University also strongly recommends that all
incoming students, especially freshmen living in the residence halls,
receive the meningococcal
vaccine.

What
to bring to campus
- Prescription medications (with written prescription
from your physician if possible)
- Over the counter medications: Tylenol, Advil, cold
medications, vitamins, sunblock (Our pharmacy stocks many of these
items, often at lower cost than the area stores; we can also be
bill your student account. Click here

- Eyeglasses (even if you wear contacts)
- Thermometer
Medical Advice
Our nursing staff is available 24 hours a day, 7 days
a week, when school is in session. Please call (863-1330) anytime
for questions or advice on what to do when you are feeling ill or
have a health related question.
