Dr. Gatsonis is a leading authority on the design and analysis of clinical trials of diagnostic and screening modalities and has extensive involvement in methodologic research in medical technology assessment and in health services and outcomes research. He is Group Statistician of the American College of Radiology Imaging Network (ACRIN), a NCI funded collaborative group conducting multi-center studies of diagnostic imaging and image-guided therapy for cancer. In his ACRIN work, Dr Gatsonis is the chief statistician of the Digital Mammography Imaging Screening Trial (a national study comparing digital to film mammography) and is also the chief statistician for ACRIN's arm of the National Lung Screening Trial (NLST). Dr Gatsonis was the lead statistician of the International Breast MRI Consortium and of the Radiologic Diagnostic Oncology Group (RDOG).
Dr. Gatsonis has served on numerous review and advisory panels, including the HSDG Study Section of the Agency for Health Care Policy Research, panels of the Center for Devices and Radiological Health of FDA, the Commission of Technology Assessment of the American College of Radiology, Data Safety and Monitoring Boards for NINDS and the VA, the Immunization Safety Review Committee of the Institute of Medicine. He is a member of the steering group of the STARD project (Standards for Reporting of Diagnostic Accuracy), which recently published the first comprehensive checklist for reporting the results of diagnostic test evaluations. He is also co-convenor of the Screening and Diagnostic Tests Methods Working Group of the Cochrane Collaboration and a member of the steering group of the Cochrane Diagnostic Reviews initiative, which is developing the methodologic and organizational infrastructure for the inclusion of systematic reviews of diagnostic accuracy in the Cochrane Library.

Research Description

Statistical methods for Diagnostic Test Evaluation

Dr. Gatsonis' current and recently completed work in this area addresses problems arising in the design and analysis of clinical studies of diagnostic modalities. Topics include, Bayesian methods for the design of multi-reader, multi-modality studies (with Fei Wang, Boston University), Two-phase designs for estimating and comparing ROC curves (with Mei –Hsiu Chen, Brown University), Group-sequential designs for ROC analysis (with X-H (Andrew) Zhou, University of Washington), Studies of the predictive value of diagnostic tests (with Shang Ying Shiu, Brown University), Bayesian methods for equivalence studies (with Kostas Kalogeropoulos), Designs of studies evaluating the outcomes of diagnostic tests (with Bo Lu, Ohio State University), and Methods for the evaluation of imaging biomarkers.

Multi-center clinical studies of diagnostic and screening tests

Since 1999 Dr. Gatsonis has been Group Statistician of the American College of Radiology Imaging Network (ACRIN), a NCI funded collaborative group conducting multi-center studies of diagnostic imaging and image-guided therapy for cancer (http://stat.brown.edu/section/ACRIN/ and www.acrin.org ). In addition to his broader scientific and managerial role in ACRIN, he is the lead statistician in several of the network's studies. In particular, he is the lead statistician of the Digital Mammography Imaging Screening Trial (DMIST—a national study comparing digital to film mammography) and also the lead statistician for ACRIN's arm of the National Lung Screening Trial (NLST—a national study comparing helical CT to x-ray in screening for lung cancer).

Before the launching of ACRIN, he was the lead statistician of the Radiologic Diagnostic Oncology Group (RDOG) , which pioneered multi-center studies in radiology, and of the International Breast MRI Consortium.

Meta-analysis of diagnostic accuracy studies

Dr. Gatsonis' methodologic work in this area introduced and developed the Hierarchical Summary ROC (HSORC) approach to meta-analysis of studies reporting estimates of the accuracy of tests. A primer for systematic reviews of diagnostic accuracy (co-authored with Prashni Paliwal, Yale University) will appear in AJR shortly.

He is co-convenor of the Screening and Diagnostic Test Methods Working Group of the Cochrane Collaboration ( http://www.cochrane.org/sadt/index.htm ) and a member of the Steering Group which oversees the development of the handbook and infrastructure necessary for launching the Cochrane Diagnostic Reviews. He serves as a co-editor of the Handbook and, together with Petra Macaskill (University of Sydney), a lead author of the chapter on statistical methods.

The International Breast MRI Consortium (IBMC) was an international, NCI-funded consortium organized to evaluate the role of MRI in breast cancer. The consortium conducted two multi-center studies, a large, prospective evaluation of breast MRI for women with a suspicious mammogram, and a pilot study of the potential of breast MRI in screening women at high risk for breast cancer. The Biostatistics and Data Management Center of IBMC was directed by Constantine Gatsonis and was operated jointly by the Brown University Center for Statistical Sciences and the ACR's Data Management Center in Philadelphia.

Staging Breast Cancer with Positron Emission Tomography
University of Michigan Subcontract,
NCI funding period: 09/30/1995-07/31/2001
This subcontract provided support for the Biostatistics Center of a multi-center prospective trial to assess the accuracy of PET in staging patients with newly diagnosed breast carcinoma.
