U.S. Citizens and permanent residents will need to complete the FAFSA form, ideally 2-weeks after your tax return has been filed with the IRS. You can complete the FAFSA online at www.fafsa.ed.gov. You will need to obtain a PIN to sign the form electronically. The Alpert Medical School code is E00059. |

Students who wish to be considered for institutional funding (AMS scholarships and loans) must complete a Need Access Application in addition to the FAFSA. The Need Access application must include: parents' income and asset information, and the student's income and asset information. If a student is married, they must include their spouse's income and asset information. If parents are divorced or separated, both student's custodial and non-custodial parent must complete the Need Access Application. This information is password-protected and can be accessed confidentially by three different parties. |

Students must provide a signed copy of their parents' tax return(s), including all schedules and W-2 form (s). In the case of divorced parents, the custodial and non-custodial parent must both submit a signed copy of their tax return including all schedules and the W-2 form(s). |

This form is used to communicate changes in family income and assets, and out-of-pocket medical and dental expenses that a student would like to include in their cost of attendance. It guides students and their families through the process of explaining circumstances thoroughly and providing appropriate documentation that will be needed in addition to the form. |

This form is used to alert the financial aid office of any upcoming enrollment changes due to fellow status, study away, or leave of absence. Note: completing this form for the OFA does not replace academic adviser approval for time away. |

Financial aid applicants who are involved in a business, or whose families are involved in a business, may be asked to complete this form. The form will be requested at the point the application is reviewed. |

College Enrollment Verification |
Financial aid applicants who report on the FAFSA or Need Access forms that other family members will be enrolled in a postsecondary institution are required to provide verification of enrollment on an annual basis. The OFA will send a request for completion of this form to the appropriate students in late September. |

Financial Aid Transcript |
Students pursuing a graduate degree at another institution during a leave of absence will be required to provide a financial aid award letter(s) detailing the aid received from the institution during their attendance. This document will satisfy the FAT requirement and allow AMS to develop an aid package once a student returns. |
