Forms Required for all Sports Campers at check-in

All Campers are required to submit the Sports Camps Acceptance Form , Medical Authorization Form and if an overnight camper the Immunization History Form. All forms will be collected at camp check in. Without these forms completed in their entirety, campers will not be permitted to participate in any camp activity.

Brown University Summer Sports Camps

NOTE: If your camper needs medical or disability accommodations, including significant food allergies (such as gluten, wheat, milk soy, or eggs) that require special meals, please fill out the Disability and Medical Accommodations Form and/or Guest Allergy Form below.  Please be aware that all accommodation requests will be considered; however, there may be limits as to what can be provided without sufficient notice. Please note forms are only accessed in the case of an emergency.

Forms Required for all Sports Campers at least 2 weeks prior to check-in if applicable

These forms should be returned directly to SEAS office at least two weeks prior to the start of camp see form for fax and address).

If you are attending any of Brown's Pre-College Programs and have already submitted Immunization History and Medical Authorization Forms, you do not need to re-submit. Please submit only the Sports Camps Acceptance Form at your camp check in.

If your camper has any information or concerns that our athletic training staff should be aware of throughout your child’s time at our camp, please contact Lisa Vidal at 401-863-2231 in order to have our staff better prepared. When contacting, please be sure to include: the camper’s name, the name of the camp and the dates of attendance.
