Religious Studies Concentration Forms:
When are forms due?

All forms can be found at the bottom of the page.

I.  Spring Junior Year (normally the sixth semester).

  1. Declaration of Concentration Form.  This is to be filled out by you and signed by the DUS.  One copy is yours to keep, one copy stays with the DUS, and one copy is for the Registrar.  It is your responsibility to take the third copy to the Registrar.
  2. Senior Project Information Form. This is the form where you declare whether you will do a Capstone or a Thesis, this form is due to the Departmental Administrator, Tina Creamer, in the Spring of your junior year.  To decide which project suits you best, please refer to the Capstone and Thesis description posted on the website
  3. Only for those who choose to complete and Honor's Thesis:  The Thesis Proposal is due to the Departmental Administrator, Tina Creamer, at the same time as your Senior Project Information Form.  You are responsible for obtaining the necessary signatures and approval from the faculty member supervising your Honors Thesis.  For more information on how to write the proposal and when to hand it in, refer to the Honors Thesis Proposal Guidelines posted on the website.  

PLEASE NOTE: THERE IS NO SEPARATE PROPOSAL FORM, BESIDES THE SENIOR PROJECT INFORMATION FORM, FOR THOSE WHO ARE DOING AN HONOR"S THESIS.  SIMPLY HAND IN BOTH THE SENIOR INFORMATION FORM AND THE THESIS PROPOSAL FORM AT THE SAME TIME.

CAPSTONE PROJECTS WILL REQUIRE A SEPARATE PROPOSAL FORM, IN ADDITION TO THE SENIOR PROJECT INFORMATION FORM, THAT WILL BE DUE IN EITHER FALL OR SPRING OF SENIOR YEAR.  SEE DETAILS BELOW:

II.  Fall Senior Year (normally the seventh semester). 

  1. RS Concentrators Information and Checklist.  In anticipation of completing requirements, all students, whether doing a thesis or a capstone, should download and fill out this form and submit it the Departmental Administrator, Tina Creamer, by the end of registration for the spring of your senior year.  This checklist form is available below.  
  2. If you are doing a fall capstone, your capstone proposal form and the actual capstone proposal are both due to the Departmental Administrator, Tina Creamer, by October 15th.  For details on the capstone proposal, refer to the capstone proposal guidelines posted on website.  You are responsible for obtaining the necessary signatures and approval from the faculty member supervising your capstone.
  3. Once you have completed a fall capstone, download, fill out, and obtain the necessary signatures on the capstone completion form.  

III.  Spring Senior Year (normally eighth semester)

  1. If you have done an Honor's Thesis: Once the thesis is complete, download, fill out, and obtain the necessary signatures on the Honor's Thesis Completion Form.  Check with the Departmental Administrator, Tina Creamer, for when this form is due.  
  2. If you are doing a spring capstone, your capstone proposal form and the actual capstone proposal are both due to the Departmental Administrator, Tina Creamer, March 4th.  For details on the capstone proposal, refer to the capstone proposal guidelines posted on the website.  You are responsible for obtaining the necessary signatures and approval from the faculty member supervising our capstone.
  3. Once you have completed a spring capstone, download, fill out, and obtain the necessary signatures on the capstone completion form.  Check with the Departmental Administrator, Tina Creamer, for when this form is due.
