Network Extension Request Form

This form should be used for anyone wishing to add a Gaming Console, Hub, or Switch to the campus network. 

This form is used to inform the appropriate CIS staff about your plans, and someone will get back to you regarding the request. For requests related to the wiring of taps in a building, please fill out the Wiring Change Request form.

NOTE: No personal wireless routers will be allowed in the dorms since they can interfere with Brown's wireless network. If using a personal router, the wireless capability must be disabled.

How to obtain the mac address on your gaming console

1. Tap where device is to be connected to the campus network. Please specify Building, Room, and TapID (on the label on the wall plate).
2. Who will be the owner of the device?
3. Please provide a Brown phone number for the device owner.
4. Please provide an email address at Brown for the device owner.
5. What is the MAC address of the device?
6. What type of device would you like to install?
7. What IP address range will you serve? (You cannot use Brown address space.)
8. Is there anything else you think we should know about your planned installation?
