Progress Report Submission Procedures
Expedited and Full Board Protocols

Here’s what we need from you to make a complete progress report submission:

For Full Board Protocols please provide:

• original, signed progress report form, all responses to form questions and all items requested on the form

• eleven (11) copies of the above

 

For Expedited Protocols please provide:

• Original, signed progress report form, all responses to form questions and all items

 

All progress reports must be submitted to the IRB as indicated above.  Progress reports that do not follow the required format will be returned to the investigator, as incomplete, without IRB review.

 

Submit all progress reports to the Research Protections Office, Box 1986, Attn:  IRB Coordinator, in sufficient time to meet the agenda deadline.
