- Summer Abroad Acceptance Form
- General Travel Waiver
- Immunization History Form (non-Brown students only)
- Health Insurance Waiver Form (only if you are waiving the Brown student health insurance plan)
- Disability and Medical Accommodations Form (as applicable)
- Scanned and emailed clear copy of your passport’s photo page
- Copy of your travel itinerary (email to summerabroad@brown.edu)
- Paris Housing Questionnaire (to match you with an appropriate host family)

Certificate of Eligibility I-20 Request Form/ F-1 Visa
Students of the Sciences Po will be expected to obtain an F-1 student visa from the US Embassy in Paris in order to participate in the Brown-based portion of the program. Please visit our website for more information on obtaining the F-1 visa and downloading the Certificate of Eligibility I-20 Request Form.
Sciences Po will also need to provide you an official letter with proof of funding for the program in order for your I-20 to be processed and to take to your consulate visa appointment.
