The Statement of Appointment (Form PHS 2271) should be submitted at the time of the trainee's appointment and is required for new appointments, re-appointments and amended appointments. Grantees must submit the PHS2271 data electronically using the xTrain system. The Termination Notice (Form PHS 416-7, also submitted via xTrain), is due within 30 days of the end of training grant support and is required for all students terminating from a training grant.
