Required Forms These forms are required as part of the 2018-2019 financial aid application process

Intent to Apply for Financial Aid

This form indicates your financial aid application plans for the 2018-2019 academic year.  Please upload this document as MDFED, Fed Aid Only App, into SSB.  All applicants must complete this form.

Free Application for Federal Student Aid (FAFSA)

U.S. citizens and permanent residents will need to complete the FAFSA form, ideally 2-weeks after your tax return has been filed with the IRS. You can complete the FAFSA online at www.fafsa.ed.gov. You will need to obtain a PIN to sign the form electronically. The Alpert Medical School code is E00059.

AMS Institutional Aid Application 

All incoming and returning students applying for institutional aid must complete an Alpert Medical School Institutional Financial Aid Application.

2016 Student Non Filer Form

If student is not required to file a federal tax return for 2016, he must submit a Student Non-Filer Form.

2016 Parent Non Filer Form

If parent(s) is not required to file a federal tax return for 2016, she must submit Parent Non-Filer Form.

2016 Parent Federal Income Tax Returns

Student must provide a signed copy of his parents' tax return(s), including all schedules and W2 form (s). In the case of divorced parents, both parents must submit a signed copy of their tax return including Schedules C and E and W2 form(s).

Special Circumstances Form

This form is used to communicate changes in family income and assets, and out-of-pocket medical and dental expenses that a student would like to include in their cost of attendance. It guides students and their families through the process of explaining circumstances thoroughly and providing appropriate documentation that will be needed in addition to the form.

2017 Student Non Filer Form If student is not required to file a federal tax return for 2017, he must submit a Student Non-Filer Form.
2017 Parent Non Filer Form If parent(s) is not required to file a federal tax return for 2017, she must submit Parent Non-Filer Form.

Cost of Attendance Adjustment Form

Students must provide a detailed statement and proof of payment in addition to this form to request a COA adjustment for unforeseen school related expenses.

Business Information Form

Financial aid applicants who are involved in a business, or whose families are involved in a business, may be asked to complete this form. The form will be requested at the point the application is reviewed.

Request to Waive Parent Information

AMS requests that both parents provide income and asset information on the FAFSA.  If there has been very limited or no contact with a parent for a significant period of time or perhaps, a parent’s whereabouts is unknown, the student can request that parent data be waived from the consideration in the financial aid application.

Digital Signature Instructions A digital ID is needed to sign financial aid documents.  Please follow the instructions to create a safe and secure digital signature.
