Required Forms These forms are required as part of the initial financial aid application process

Free Application for Federal Student Aid (FAFSA)

U.S. citizens and permanent residents will need to complete the FAFSA form, ideally 2-weeks after your tax return has been filed with the IRS. You can complete the FAFSA online at www.fafsa.ed.gov. You will need to obtain a PIN to sign the form electronically. The Alpert Medical School code is E00059.

AMS Institutional Aid Application 

All incoming and returning students applying for institutional aid must complete an Alpert Medical School Institutional Financial Aid Application.

2015 Student Non Filer Form

If student is not required to file a federal tax return for 2015, he must submit a Student Non-Filer Form.

2015 Parent Non Filer Form

If parent(s) is not required to file a federal tax return for 2015, she must submit Parent Non-Filer Form.

Parent 2015 Federal Income Tax Returns

Student must provide a signed copy of his parents' tax return(s), including all schedules and W-2 form (s). In the case of divorced parents, the custodial and non-custodial parent must both submit a signed copy of their tax return including all schedules and the W-2 form(s).

Special Circumstances Form

This form is used to communicate changes in family income and assets, and out-of-pocket medical and dental expenses that a student would like to include in their cost of attendance. It guides students and their families through the process of explaining circumstances thoroughly and providing appropriate documentation that will be needed in addition to the form.

2016 Student Non Filer Form If student is not required to file a federal tax return for 2016, he must submit a Student Non-Filer Form.
2016 Parent Non Filer Form If parent(s) is not required to file a federal tax return for 2016, she must submit Parent Non-Filer Form.

Cost of Attendance Adjustment Form

Students must provide a detailed statement and proof of payment in addition to this form to request a COA adjustment for unforeseen school related expenses.

Business Information Form

Financial aid applicants who are involved in a business, or whose families are involved in a business, may be asked to complete this form. The form will be requested at the point the application is reviewed.

Request to Waive Parent Information

AMS requests that both parents provide income and asset information on the Need Access Form. If the non-custodial parent’s whereabouts are unknown, if there has been very limited or no contact with the non-custodial parent for a significant period of time, or if there are certain other extenuating circumstances, the student can request that the non-custodial parent data be waived from the consideration in the financial aid application.
