Required Forms Description

Free Application for Federal Student Aid (FAFSA)

U.S. citizens and permanent residents will need to complete the FAFSA form, ideally 2-weeks after your tax return has been filed with the IRS. You can complete the FAFSA online at www.fafsa.ed.gov. You will need to obtain a PIN to sign the form electronically. The Alpert Medical School code is E00059.

Need Access Form

Students who wish to be considered for institutional funding (AMS scholarships and loans) must complete a Need Access Application in addition to the FAFSA. The Need Access application must include parents' and student's income and asset information.  If student is married, she must include her spouse's income and asset information.

If parents are divorced or separated, both student's custodial and non-custodial parent must complete the Need Access Application. This information is password-protected and can be accessed confidentially by three different parties.

AMS Application Form

All incoming and returning students applying for institutional aid must complete an Alpert Medical School Financial Aid Application. (Be sure to print, sign and return to the Office of Financial Aid.)

Student Non Filer Form

If student is not required to file a federal tax return for 2013, he must submit a Student Non-Filer Form.

Parent Non Filer Form

If parent(s) is not required to file a federal tax return for 2013, she must submit Parent Non-Filer Form.

Parent 2013 Federal Income Tax Returns

Student must provide a signed copy of his parents' tax return(s), including all schedules and W-2 form (s). In the case of divorced parents, the custodial and non-custodial parent must both submit a signed copy of their tax return including all schedules and the W-2 form(s).

Other Forms Description

Special Circumstances Form

This form is used to communicate changes in family income and assets, and out-of-pocket medical and dental expenses that a student would like to include in their cost of attendance. It guides students and their families through the process of explaining circumstances thoroughly and providing appropriate documentation that will be needed in addition to the form.

Enrollment Change Form

This form is used to alert the financial aid office of any upcoming enrollment changes due to fellow status, study away, or leave of absence. Note: completing this form for the OFA does not replace academic adviser approval for time away.

Business Information Form

Financial aid applicants who are involved in a business, or whose families are involved in a business, may be asked to complete this form. The form will be requested at the point the application is reviewed.

Request to Waive Parent Information

AMS requests that both parents provide income and asset information on the Need Access Form. If the non-custodial parent’s whereabouts are unknown, if there has been very limited or no contact with the non-custodial parent for a significant period of time, or if there are certain other extenuating circumstances, the student can request that the non-custodial parent data be waived from the consideration in the financial aid application.
