Impacts of an Outpatient Pediatrics Social Determinants of Health Intervention and Referral Program

The World Health Organization identifies that social determinants of health (SDH) such as income, food, and housing stability can have a greater impact on and account for 30-50% of health outcomes. It is becoming increasingly recognized that screening for SDH and providing appropriate referrals and interventions are essential for promoting health and well-being. This research aims to quantitatively evaluate the impact of Connect for Health (CFH), an outpatient SDH intervention and referral program in Rhode Island. In addition to collecting and describing sociodemographic data for CFH clients, the study attempts to identify the barriers that hinder access to resources. The results of the study can directly impact the CFH workflow, improve the quality and success of client communication, and identify potential interventions that may help mitigate the barriers experienced by the CFH clientele. The findings can be used to develop programs for community engagement between university student volunteers, healthcare institutions, and vulnerable populations, and can serve as crucial support when advocating for additional funding with key stakeholders.

Lana Nguyen ‘25, a native of San Francisco, is pursuing a Health and Human Biology degree and an Entrepreneurship certificate. Growing up in a low-income neighborhood, Lana developed a passion for community service and supporting vulnerable populations. Combining medicine, entrepreneurship, and public health, Lana’s research interests focus on developing sustainable interventions for hard-to-reach populations, including those affected by HIV, HCV, and overdoses. As a recipient of multiple research awards, Lana has worked as a research assistant at UCSF, Brown’s School of Public Health, and Hasbro Children’s Hospital on projects that center around social determinants of health. On campus, Lana serves as a Patient Advocate with Connect For Health and an HIV Committee Director with Clinica Esperanza. She is also a Founder of SupplyHopeInfo and a Program Director of Young Entrepreneurs Program, both of which promote higher education. Lana aims to develop research projects that address the needs of vulnerable populations while integrating their voices, experiences, and stories into research design and execution.
